How it works

1. My Lab — name your incubators, media, oil, consumables, equipment and techniques once. Everything else in the sheet draws from that list, so a CAPA names the exact unit — “Incubator 2 — K-Systems G210” — not just “incubator”.
2. The event — what happened, what you observed, how serious, when you first noticed and when it recovered.
3. What you changed — everything you tried while troubleshooting, roughly dated, each marked as having helped, not, or unsure. This is the part you actually remember.
4. The CAPA — written for you — a complete, audit-ready form: problem statement, timeline, a proposed root cause for you to confirm or overrule, corrective and preventive action, the factors you ruled out, a 30-day effectiveness check and sign-offs. Print to PDF.

Why this order

Classic CAPA forms open with “state the root cause” — the one thing a busy embryologist doesn't yet know. Starting instead from what you changed turns a week of trial and error into a documented investigation. Nothing is invented; the suggested cause is yours to confirm or rewrite.

• The KPI dip that triggers a CAPA comes from the EmbScore Monthly Workup; the recovered value is your effectiveness evidence.
